Health-Related Social Needs (HRSN)

Being healthy requires more than regular check-ups with a doctor, having access to non-medical related necessities such as secure housing, healthy foods, and air conditioning, are equally just as important to maintain your health. Certain Oregon Health Plan (OHP) members may be eligible for extra non-medical services through a new program called health-related social needs (HRSN). This name refers to barriers to health such as housing or food insecurity that may affect our well-being.

What HRSN Services does Alliance provide?

Currently, HIV Alliance can assist our clients with HRSN Rental Tenancy Supports and Outreach to additional resources

Who qualifies for HRSN Housing Supports?

** Not all OHP members will qualify for HRSN housing benefits. HRSN housing benefits are not meant for emergencies.**

To qualify for rent or utilities assistance, OHP members must:

  • Be currently renting
  • Need support staying housed
  • Have an income that is 30% or less than the median income in their area
  • Lack sufficient resources or support networks to prevent homelessness
  • Have a clinical risk factor

How do I access HRSN Support?

As a part of Alliance’s all-encompassing services, we can apply with you as your community partner. You can ask your case manager to connect you to the HRSN team, if you think you meet the eligibility. Not sure if you will qualify? We are here to help you discuss your options.​​

​​​If you’re applying for rental assistance, be prepared to have this paperwork ready:

  • Your Oregon Health Plan ID card
  • W9
  • A lease or rental agreement
  • Proof of income for the past two months like pay stubs or social security
