Supportive Housing

The Oak Program for Housing is The Alliance for Community Wellness’s agency supportive housing program. Funded through a contract with the Oregon Health Authority, the program launched in 2020 in Lane County and currently serves Lane, Douglas, Josephine, Jackson, and Marion counties. The Oak Program offers housing subsidy and financial assistance combined with wraparound supportive services to individuals who are most at risk due to their housing instability, behavioral health, and/or substance use. The program follows the evidence-based “Housing First” model, supporting the concept that permanent housing is the prerequisite for individuals to pursue their personal goals and improve their quality of life.

Intensive, wrap-around supportive services are delivered to Oak Program clients through a care team, often in the home or meeting people where they’re at to reduce client barriers to care. This team is comprised of a Supportive Housing Navigator, Housing RN Medical Case Manager, Peer Support Specialist, Resident Services Specialist, and a Behavioral Health Therapist, who collectively support clients in gaining both stable housing and independent living skills with the ultimate goals of increased viral suppression and overall wellness.

1. How do I determine my eligibility or make a referral to the Oak Program?
