Patient Navigator Assistant/Intake Coordinator

DEPARTMENT:  Client Services           

REPORTS TO:    Client Services Pilot Programs Manager and Client Services Manager

WORK SITE:      Medford




  • Current FTE: 0.5 FTE or 20 hours/week
  • Salary range: $14/hour
  • Vacation time and discretionary leave prorated to FTE

  • This position is hourly and non-exempt, i.e. subject to state and federal wage and overtime requirements



This is a dual position that will provide administrative support to the Patient Navigator and Intake Coordination for people living with HIV.

The position assists the Patient Navigator in scheduling client appointments, processing program referrals, conducting community resource and housing searches, and general program coordination.  This position works directly with the Patient Navigator to assist in conducting the program.

The Intake Coordinator is the first contact for new clients referred to the Care Coordination Center.  This position conducts the initial screening with all new clients, conducts the bi-annual phone screening with all clients, assists in obtaining required documentation, scheduling the first appointment for all new clients with a Care Coordinator and with a Medical Case Manager to complete their assessment process and refers to the Care Coordinators and Medical Case Managers as appropriate when clients need additional assistance after the annual screening is completed.



  • Receive and respond to referral requests
  • Maintain up-to-date HIV knowledge; provide HIV education to clients
  • Conduct searches for community resources and housing
  • Obtain Authorization for Release of Information between HIVA and Medical Provider
  • Schedule client visits with the Patient Navigator
  • Provide direct one-on-one and phone screening
  • Schedule intakes and bi-annual screening
  • Schedule Medical Case Manager and Care Coordinator assessments as needed
  • Maintain up-to-date and accurate client records
  • Participate in necessary meetings
  • Assist with all necessary reports
  • Participate in required training
  • Comply with all applicable standards, guidelines, laws & administrative rules
  • Other duties as defined by Director and Ryan White Principles and Standards



  • Ability to schedule for several staff members, both on site and off site
  • Ability to communicate effectively verbally and written with client, staff and partners
  • Excellent problem solving and organizational skills
  • Experience with chart notes, file maintenance
  • Experience with data entry and data quality
  • Computer skills, including Excel and Word
  • Good follow through and task management skills
  • Ability to multitask
  • Ability to work in busy and dynamic environment
  • Ability to work with diverse populations
  • TB test (provided at agency)
  • Flexible hours required, including some evenings and weekends
  • Ability to pass agency criminal background check and drug screen
  • Valid Oregon Driver’s License
